Sports Physicals for JE Students - Grades 7-12
These physicals are not for college admission, working papers or for youth football. Wednesday, July 15, 2009 Saturday, August 1, 2009 Wednesday, August 26, 2009 9:00 am – 12:00 Noon At the JE High School, Hamilton Road
Nurses Office Summer sports physicals will be done at the High School on the above dates. All students must bring physical form (download here), completely filled out and signed by a parent/guardian or physical will not be done. If you have a copy of your child’s immunization record please attach it to the physical form for the physician to review.
These physicals will also meet requirements for State mandated 7th and 10th grade physicals. Additional Forms will be available at High School Main Office or online at the Jordan-Elbridge Website www.jecsd.org. If your child had a recent physical done by your private physician, please send it or drop it off at your child’s 2009-2010 school as soon as it is done.
Jordan-Elbridge has a long, proud and rich tradition in sports. The mascot “Eagle” represents pride, strength, wisdom and character.
With three campuses in close proximity, the District has well-maintained playing fields and indoor facilities for sports at every level for boys and girls.
Jordan-Elbridge has had its share of winning teams and scholarship athletes. At the same time, JE’s smaller size allows many students to participate in athletics.

Every athlete must have a sports physical before they can participate in a sport. The school provides FREE physicals during the summer months. This form must be filled out and signed by a parent before an athlete can receive one of those FREE physicals. If you choose to have a physical done by your own physician, you must have the physician fill out this form. If you use your own physician, it is the athlete's responsibility to get a completed, signed copy of this form to the school nurse before the season starts.

The Athletic Hall of Fame Committee accepts nominations throughout the year. The award is given at the Annual Spring Athletic Banquet.
